    Before filling out the below form, please note that this is intended to notify me of the days and times that you are not at school, at school activities, etc. and are available to work.  This form is not a means to design your work schedule. Enter your most open availability and I will not exceed your preferred-max hours as noted. 

    Requesting time off is the appropriate method for short term needs or one-time vacations, etc.
